The application can be accessed from the link below. Upon approval you can place your order online, via email, telephone or fax. Our minimum initial order is $500. Follow up orders require a minimum of $300. Please note that we are sorry but wholesale orders are not eligible for exchanges or refunds. All new orders are payable upon shipment. We accept checks, Visa, MC and Amex. We ship USPS Priority mail or UPS Ground, both take 3-7 days. UPS 2nd day air or International shipping available upon request.
